Taking the pressure away from you

The disruptive impact of the COVID-19 pandemic has continued throughout the summer months and as we draw closer to winter, the NHS is faced with the ever-increasing pressures of reduced capacity and a rising backlog of care.

Thornbury Community Services (TCS) work collaboratively and flexibly with hospitals and care providers to accelerate the transition of individuals in acute hospital settings back into the community. We take the pressure away, enabling you to have longer-term, more strategic conversations with families and focus more time and resources on achieving NHS priorities.

Through our Rapid Discharge Service, we can facilitate the setup of nurse-led care packages within 48 hours for individuals with a range of complex conditions including:

• Acquired brain injury

• End of life
