We carefully match the right healthcare professionals into care packages to meet the unique and specific requirements of our clients. Our robust clinical governance and infrastructure allow us to establish a fully managed care team within 48 hours, including a quality, clinically excellent assessment, and care plan. Our integrated care plans are tailored to meet the needs of each client and undergo robust clinical risk management from our highly skilled team on an ongoing basis.

Our Rapid Discharge Service

Working in partnership with our sister company, Pulse Nursing at Home (part of Acacium Group), we created our Rapid Discharge Service that focuses on accelerating individual’s transitions from acute hospital settings back into the community. We focus on getting people back into their own homes quickly and safely, empowering them to achieve their personal goals and aspirations.

This was the case for Kaso, a young man living with an acquired brain injury. The hospital where Kaso was staying were down to its last two ICU beds, and TCS needed to build a bespoke and robust care plan with a highly skilled and dedicated care team in 72 hours; to get Kaso home in time to celebrate his 15th birthday with his family.

Gemma has dedicated the past nine years of her career specialising in complex care, supporting children and adults in the community, including those living with birth injuries, acquired brain injuries, spinal injuries, home parenteral nutrition, long-term invasive ventilation and complex airway management. Within her current role at TCS, Gemma oversees clinical leads supporting home care packages to the most clinically complex and vulnerable individuals whose high-level needs often require 24/7 care.
