Job Title: Community Care Coordinator – Learning Disability and Autism Service
Company: Independent Clinical Services
Division: Thornbury Community Services
Hours: Monday – Friday, shits on a rota basis

An exciting opportunity has arisen to join our Learning Disability and Autism Service as a Community Care Coordinator. The LD&A Care Coordinator is fully responsible for completing team rotas to ensure that the care delivered is of the highest standard. You will work with all internal teams to facilitate patient centred care that enriches the quality of our patients lives. 

The ideal candidate will have:

Good level of basic education
Numerate & literate
Strong organisational skills
Strong interpersonal skills
Good IT skills
Ability to manage a database & keep records up to date
Good telephone manner
Experience of working to deadlines and targets
Self-motivated and driven, ability to work without supervision   
Ability to work as part of a team                 
Professional manner   
Enthusiastic and committed to the business objectives of the Company   
Ability to work in a busy environment
Possess excellent written and spoken communication skills
Have experience in customer service and rota management 
Be driven to exceed set goals and targets and maximise client and nurse satisfaction 

We are looking for someone who excels at customer service, who has the sensitivity to liaise with our families and have a naturally professional nature that is maintained when speaking to clients and care staff alike. An understanding of the Learning Disability and Autism Service is desirable but not essential.
