Long-term domiciliary care provided to clients in the West of England

Overview 

From 2017 to 2019, we partnered with a council in the West of England to deliver tailored domiciliary care, primarily supporting elderly clients with complex health needs and cognitive impairments. What began as a single 30-minute visit within a 4-hour care window for one client quickly evolved. As demand grew to support more clients, we scaled up our service, deploying 4 to 6 healthcare professionals to carry out multiple 30-minute visits within the same timeframe. This approach not only expanded our reach but also offered a more cost-effective solution for the funder, without compromising on quality of care.

Detailed planning and schedules to ensure seamless care visits

Ahead of providing care to clients in the West of England, our teams carefully mapped client locations and assessed individual care needs, allowing us to optimise visit schedules, reduce travel time, and deliver cost-effective support without compromising on quality.

Structured inductions provided to all healthcare professionals

Every healthcare professional underwent a comprehensive, tailored induction before delivering care to our clients in the community. This ensured they were fully aligned with the expectations of both our clients and the council, equipping them to provide compassionate, high-quality support from day one.

Provided 24/7 operational support to the healthcare assistants

Our dedicated operations team provided round-the-clock support to the care professionals, ensuring they had immediate access to guidance and information whenever needed. Whether it was shift details or specific client circumstances, the team were available 24/7.

Maintained the same staff to provide continuity to our clients

To ensure continuity of care, we prioritised assigning the same skilled healthcare assistants to each and every client whenever possible. This consistent support helped clients feel more at ease, fostering a sense of safety, familiarity, and comfort in their own homes.

Created daily rotas to ensure high-quality care and support

We designed clear, consistent daily rotas to keep care delivery smooth and structured. These schedules empowered our healthcare assistants to provide safe, effective person-centred support, ensuring every client received the highest standard of care.

Adhered to the care plans provided to ensure we met client’s bespoke needs

We closely followed the care plans provided by the funder to ensure every client received personalised, high-quality support. Through thoughtful planning and attention to detail, our healthcare assistants delivered care that was tailored to each individual’s unique needs and preferences.

Outcome

Over the course of our 17-month partnership with the council in the West of England, our dedicated healthcare assistants delivered an impressive 3,081 domiciliary care shifts to adults and elderly individuals across the region.

This collaboration enabled us to provide tailored, high-quality support, empowering clients with diverse care needs to live safely, independently, and with confidence in their own homes and communities.

We also received insightful feedback from one of our healthcare assistants, whose experience in supporting the clients offers a valuable perspective on the impact of our service.
