With an extensive database of nurses from a variety of specialist backgrounds we can support clinical commissioning groups (CCG’s) to reduce their back-log of continuing healthcare assessments.
Our specially selected nurse teams can complete your continuing care nursing assessment documentation, submit to your team for verification and support the nursing elements of multidisciplinary team to complete Decision Support Tools (DST) within the national framework for NHS continuing healthcare and NHS funded nursing care.

Case study
We were approached by a paediatric commissioner who was experiencing shortages in their Continuing Healthcare (CHC) team and did not have the nurse assessors to carry out the CHC assessments for their area.
We met with the CHC team to assess their requirements and planned to help prevent a back log of assessments, while they recruited more nurses for their teams.
We sourced a pool of appropriately skilled and experienced nurses, who were each given training on how to conduct and write up the assessments as per the CCG’s requirements. Our nurses supported with the completion of yearly reviews for children and young people, ensuring their health needs were being met and the current support package was still appropriate.
On an ongoing basis, the CCG provides us with the assessment details of their patients and our operations team coordinate assessment bookings with our nurses. We use our bespoke software and maintain the highest standards of operational procedures, to ensure all data received and communicated is secure and promotes patient and client confidentiality.
After assessment, the documentation is written up by our nurses and audited by our in-house teams.
